MyChart Questionnaires
If your patients have a MyChart account (formerly "MyAHS Connect"), you can build and send them questionnaires prior to their visit. This allows patients to contribute to the medical record and saves your staff time collecting the information. All the data entered becomes part of the patient record automatically.

Patient Care Preferences
Social Factors
History Questionnaires (personal and family history)
When building specialty-specific questionnaires, there are a few things to consider.
Content should reflect a provincial approach
Questions should not be for patient satisfaction or quality improvement purposes.
Third-party and copyrighted questionnaires must have appropriate permission prior to use.
New questionnaires should follow the usual enhancement request process. This means that there should be a JIRA enhancement request. The relevant area council is responsible for creating and approving the content. Build work can then be done by a Physician/Clinician Builder. A configuration analyst or application lead will be responsible the change control process.
